Supported Living

We provide a tailored approach to supported living. The service user will have the right to his/her personal tenancy agreement and to have exclusive access to their bedroom or some part of the property in shared tenancies.

 

The service user will be fully involved in the structuring of his/her individual support plan. The actual support received will have the aim of supporting the individual to maintain and develop maximum levels of independence within his/her own home and community.

 

 

OmegaXConcept offer placements to young people leaving care between the ages of 16 and 24 years old. The following packages of care are offered:

 

Low level support package

Accommodation and 6 Hours Support

Medium Level Support Package

Accommodation and 10 Hours Support

High Level Support Package (Including Emergency Placement)

Accommodation and 15 hours Support (subject to review)

 

These packages of support include developing domestic/life skills social skills behaviour management; advocacy and liaison with professionals; help in managing finances; emotional support; help in establishing personal safety and security; risk assessment and liaison with professionals and other agencies/organisations.

In addition to the above packages OmegaXConcept are able to offer individualised support packages as requested by the referring officer to suit the needs of each young person referred. Please call us for further information and assistance.
