Introduction

 

The service aims to provide a step-forward pathway for young people (16 – 24 yrs) moving forward into transition from children’s services, their family home and educational placements; it will provide appropriate services for those who require some guidance/supervision in working towards independent living.

 

The service will support people in developing the skills required to live as independently as possible. This includes:

  • Provision of in-house training to support the development of independent living skills.

  • Close working with local colleges.

  • Supporting young people in working towards voluntary work, employment, leisure pursuits.

  • Review individual objectives and programme plans.

 

Length of stay will be planned in association with care coordinators, support teams and families, with a view to ultimately transitioning into independent living placements.

 

 

 

 

 

 

 

Supported Living
 

​​We provide a tailored approach to supported living. The service user will have the right to his/her personal tenancy agreement and to have exclusive access to their bedroom or some part of the property in shared tenancies.

 

The service user will be fully involved in the structuring of his/her individual support plan. The actual support received will have the aim of supporting the individual to maintain and develop maximum levels of independence within his/her own home and community.

​Our Staff

 

Our team consists of 7 Staff including a qualified mental nurse and a psychologist. We have room to accomodate 4 young adults. Staff  provide daily reports and can design unique care packages to suit the young adults needs.
